1. General Eligibility

To qualify for transportation services, clients must meet the following criteria:

Be at least "18 years of age", or accompanied by a legal guardian

Reside within the designated service area (e.g., Maricopa, AZ and surrounding regions)

Require "non-emergency transportation" for medical appointments, errands, events, or other approved purposes

Be able to safely enter and exit the vehicle with or without assistance

2. Medical & Mobility Considerations

Clients with mobility aids (wheelchairs, walkers, etc.) must disclose this at the time of booking

Transportation is not suitable for individuals requiring emergency medical care or ambulance-level support

Drivers are not authorized to administer medical assistance

3. Behavioral Expectations

- Clients must be respectful and cooperative with drivers and staff

- Disruptive, aggressive, or unsafe behavior may result in service denial or termination

- Repeated violations may lead to permanent disqualification from future services

4. Documentation

- Clients may be asked to provide identification and proof of appointment or service need

- Medicaid or insurance-based riders may be required to submit authorization or referral documentation

5. Service Limitations

Service availability is subject to scheduling capacity and geographic coverage

Amity Non-Emergency Transportation LLC reserves the right to refuse service if eligibility criteria are not met
