1. Service Overview
Amity Non-Emergency Transportation LLC provides scheduled transportation services for individuals requiring non-emergency medical or personal travel. All services are subject to availability and confirmation.
2. Eligibility
Clients must:
Be at least 18 years old or accompanied by a legal guardian
Provide accurate contact and appointment information
Disclose any mobility or medical needs relevant to safe transport
3. Booking & Cancellations
Appointments must be scheduled at least 24 hours in advance
Cancellations must be made at least 24 hours prior to the scheduled pickup time
Late cancellations or no-shows may incur a full-service charge
4. Payment Terms
All payments are subject to our [Payment Terms](#). Private pay clients must remit payment prior to service unless otherwise agreed. Late payments may result in service suspension or additional fees.
5. Conduct & Safety
Clients are expected to:
Treat drivers and staff respectfully
Wear seatbelts at all times
Refrain from smoking, eating, or disruptive behavior during transport
Amity Non-Emergency Transportation LLC reserves the right to refuse service to any individual who poses a safety risk or violates these terms.
6. Liability Disclaimer
While we take every precaution to ensure safe and timely transport, Amity Non-Emergency Transportation LLC is not liable for:
Delays due to traffic, weather, or unforeseen circumstances
Loss or damage to personal belongings during transit
Medical outcomes related to missed or delayed appointments
7. Privacy
All client information is handled in accordance with our [Privacy Policy](#). We do not share personal data without consent, except as required by law or for service coordination.
8. Modifications
